Please choose the ASQ form below appropriate for your child's age on their screening day:
ASQ:SE-2 for 2 yrs, 9 mo-3 yrs, 5 mo
ASQ:SE-2 for 3 yrs, 6 mo-4 yrs, 5 mo
ASQ:SE-2 for 4 yrs, 6 mos- 5 yrs, 5 mo
4. Complete Online forms *Select "Skyward Login" if you are an existing ISD 181 family, otherwise select "New to the District"
-Complete Step 1: Student Information
*Select "Kindergarten Enrollment" as Boundary School for any child who has yet to attend Kgn
-Complete Step 2: Family/Guardian Information
-Complete Step 3: Requested Documents
*Please attach pictures/scans of birth certificate (the novelty birth certificate from the hospital with the footprints will also work) and immunization records
-Complete Step 4: Additional District Forms
*Please complete the following:
-Final Enrollment Questions
-Minnesota Language Survey
-Child Health and Developmental History (3-5 years)
-Early Childhood Screening Consent
-Early Childhood Screening Release of Information
